Online Open Enrollment Begins May 1, 2015.

Open Enrollment for the 2015/2016 Plan Year will run from May 1, 2015 through 5:00 pm on Sunday, May 31, 2015. This year eligible employees will make their benefit elections through the District’s  (ESS) portal. With 24/7 access, employees can review benefit plan information and proceed through the enrollment process at their own convenience. Employees will also be able to review their current insurance elections via the ESS portal.

Open Enrollment is mandatory for all employees even if you are not making any changes to your insurance benefits for the upcoming plan year.  To keep the same benefits, you will need to select the "No changes" option where applicable and then "Decline coverage" for the benefits that you do not want.  For District Life, you will need to select "Make new election" if you want to update your beneficiary designations.  You will also need to select "Make new election" to elect new insurance and/or to re-enroll in the FSAs. 

Elections made during this Open Enrollment will be effective 7/1/2015.

Note: You cannot complete the Online Enrollment process unless you have a PUHSD computer LOGIN and Password. If you don’t currently have one, please contact your Asst. to the Principal or the Help Desk (41201) immediately.

Insurance providers will also visit several campuses during the month of May. In addition, Cyndy Nelke, Employee Benefits Specialist, will be visiting all of the campuses to assist with the online process.  To find a meeting, please review the 2015-2016 Open Enrollment Meeting Schedule.

Please note: you do not have to attend a meeting to complete Online Open Enrollment.

Enrollment Form Requirements

In addition to making your insurance elections Online, Short Term Disability (STD) requires you to complete a STD enrollment form if you are adding or changing your coverage. The enrollment process is not finalized until both steps are complete. The form is available below or directly from ESS during your Online Enrollment Process. Please turn in the form to Employee Benefits (CES 5th Floor or Fax#602-229-8306) no later than 5:00pm on Sunday, May 31, 2015 or your application will be disqualified.  Please note: only the first page of the application is required for decreases or increases of one level (maximum benefit $5000).  New applications and increases of two or more levels require the full form.

After completing your Online Enrollment, please print a copy of your Confirmation Page for your records. Note: you can still make changes to your elections until Open Enrollment closes at  5:00pm on Sunday, May 31, 2015

Once your elections are posted, your elections will be final and can not be changed until the 2016 Open Enrollment unless you experience a Qualifying Event.

All insurance elections must be finalized and any required supplemental documentation submitted to Employee Benefits prior to 5:00pm on Sunday, May 31, 2015.   If you do not complete Open Enrollment, you will be enrolled in the free District medical and life insurance only; any additional insurance that you may currently be enrolled in will be cancelled eff. 6/30/2015.  Thank you for your cooperation.
